Prize!

Ok, so as some of the people (people who voted on my poll) didn't want to see the prize, I thought it would only be fair if I did it this way. I'm going to put a "mystery" picture of the prize and a riddle. For those of you who don't want to know what it is, then reading the riddle might just give it away! 
The picture looks blurry, but it's not. Somehow it just looks it.

    Oh, and about that survey that I wanted to do... I do have a google acount and I put the questions that I wanted to put... BUT I have NO idea how to put it on my blog or how other people can get to it! LOL! Help! Tell me how to do it on my blog! Thanks a bunch!
